This questionnaire is about your experience of using services provided by Crossroads Care Oxfordshire. The survey is being carried out by Carers Trust who are a major national UK charity solely focused on improving the lives of carers. Crossroads Care Oxfordshire is a Network Partner of Carers Trust who work locally to support carers, giving them access to help, advice and breaks from caring.
You are being sent this survey because you have used a service provided by Crossroads Care Oxfordshire. The aim of the survey is to monitor and improve the services Crossroads Care Oxfordshire provides for carers. Your responses will help shape the future provision of support for carers in Oxfordshire. Please answer each question as fully as possible.
Taking part in this survey is voluntary. Published reports will not contain any personal details.
Who should complete the questionnaire?
The questions should be answered by you, as the carer who has accessed help and support from Crossroads Care Oxfordshire. If you need help to complete the questionnaire, the answers should be given from your point of view – not the point of view of the person helping. Your answers should all relate to support you have received from Crossroads Care Oxfordshire not for example from your local authority.

Your responses are completely confidential and anonymous. The information is being collected for Crossroads Care Oxfordshire by Carers Trust, an experienced survey provider. Carers Trust are subject to strict obligations regarding the security and confidentiality of your information and will not use your details for any other purpose unless you give us explicit permission to do so.
Information from your responses will not be released unless required by law or where there is a clear overriding public interest. Your responses will be grouped with other respondents so that we can produce an overall set of data which will highlight the key issues. This will help Crossroads Care Oxfordshire improve the services they offer.
